TEXT ORDER: The Court is in receipt of Defendant's motion for leave to appear pro hac vice. (ECF No. 8 ). The motion is deficient in that it does not: (1) indicate whether Plaintiff consents to this application as required by the undersigned's preferences; (2) include a certification from the individual seeking pro hac vice admission that is ink-signed as required Local Rule 5.2(12); (3) disclose each Federal bar in which the applicant is a member in good standing including the year of admission and the name and address of the official or office maintaining the roll of such members of its bar, or in lieu thereof certificates of good standing issued by the person or office maintaining the roll of the members of its bar; and (4) utilize the appropriate jurat, see 28 U.S.C. § 1746.
